In 2010, the Affordable Care Act created a higher demand for care, and as the number of patients seeking healthcare increased, the demand for higher-quality nursing increased as well. As a result, the Institute of Medicine initiated a goal of 80% of the nursing force obtaining baccalaureate education and higher by 2020; as of 2022, less than 62% of nurses have a bachelor’s in nursing, but the numbers continue to increase (National Academy of Medicine, 2022).

As the demand for high-quality care continues to increase, advanced practice nurses must ensure that patients are given the highest quality care possible. A global pandemic taught the world that nurses will give the best possible care regardless of the setting. During this time, the demand for home health nurses increased. Patients began going home immediately after extensive orthopedic surgeries to receive home health nursing for wound care and home health physical therapists for therapy.

Patients are being referred to home health because they have been unable to be seen in person over the last two years. It has become essential for advanced nursing positions such as clinical leaders, nurse practitioners, and nurse midwives to shift to accommodate homebound patients. Patient outcomes increase by increasing access to nursing care and education outside clinical settings.

Scope of Practice Sample Paper

Nursing Regulatory Bodies are jurisdictional governmental entities that regulate nursing practice in all 50 states, four US territories, and the District of Columbia. The National Registration Board sets the criteria for safe nursing care and issues nursing licenses (Papinaho et al., 2019). The NRB’s role does not end once a license is awarded; they continue to monitor licensees’ compliance with jurisdictional laws and take appropriate action against the licenses of nurses who have engaged in improper nursing practice.

The regulatory bodies in Michigan include The Michigan Council of Nurse Practitioners and the Michigan Board of Nursing. The Nurse Practice Act is a law enforced by each NRB in each jurisdiction. In order to keep their licenses, nurses must follow the legislation and relevant norms. The act specifies Qualifications for obtaining a license, the scope of work, Nursing titles that are permissible to use, and actions that may or may not occur if a nurse fails to obey the nursing law.

The National Council of State Boards of Nursing developed the APRN consensus model to increase job satisfaction and elevate APRN roles. The model significantly influences education and Nursing Competency as it states that: Nurse’s education must be coherent in terms of role and population focus (Buck, 2021). Secondly, APRNs can further their education and specialize but can not be licensed only in that field. I will continue to serve the veteran population as an Advanced Practice Registered Nurse (APRN). There are multiple health disparities and inequities amongst different veteran populations. Disparities by population groups include race/ethnicity, aging veterans, mental illness, women veterans, LGBTQ+ veterans, homeless veterans, and veterans who live in rural areas or who are a great distance from Veterans Affairs (VA) medical facilities (Kondo et al., 2017).

For this discussion, I will focus on the health inequities surrounding veterans and colorectal cancer (CRC) screening. CRC is the third most prevalent cancer in the United States. Veterans living in rural areas tend to have more difficulty with adhering to CRC screening. These veterans often have travel times of an hour or more to a VA medical facility (McDaniel et al., 2019). There may often be scheduling delays at VA facilities. A study by McDaniel et al. suggests that veterans living in rural areas may have lower education levels, more poverty, and less access to healthcare and, therefore, may be less likely to be screened for CRC. McDaniel et al. (2019) recommends evidence-based interventions to reduce disparities among the rural veteran population. These include mailed fecal immunochemical tests (FIT) and telehealth applications.

APRNs can advocate for vulnerable veteran populations.  APRNs working in VA Medical Centers can help combat the disparities of rural veteran populations with CRC screening by developing policies that promote high-quality CRC screening. APRNs can create change through system thinking to promote prevention and screening. Professional organizations with interests in preventing CRC and screening for CRC that APRNs can partner with include The Society of Gastroenterology Nurses and Associates (SGNA), The American Society of Gastrointestinal Endoscopy (ASGE), The Prevent Cancer Foundation, and Cancer Action Network. SGNA offers courses and networking opportunities for CRC advocacy. ASGE helps with policy by providing the committee hearing schedules for the House and Senate. The Prevent Cancer Foundation provides advocacy toolkits. The Cancer Action Network assists APRNs in influencing policy at the federal and state levels (Morrow & Greenwald, 2021).

Global perspectives of care and diversity issues for prevention and screening for just about any population are impacted by the varied differences in environment, available resources, and socioeconomic status. Health equity can only be achieved by addressing environmental and social determinants of health and eliminating disparities related to care access. Combating global health issues requires a multidisciplinary approach and involves many complex factors extending beyond the organizational level. Partnerships must be built that exchange knowledge and capabilities to address global health factors that are environmental, social, cultural, infrastructural, technological and political (Rutgers, n.d.).

Personal Worldview

My worldview is a philosophical, cognitive perspective that guides the universe and my humanity’s relationship to it. My worldview is founded on biblical dogma, scientific ideologies, and metaphysical concepts. Moreover, my worldviews are intricately linked to my principles, feelings, encounters, and postulates (Taves, 2020). My worldview also acknowledges that my patients, believers and nonbelievers, have distinct spiritual needs. Everyone, including myself, is prone to skepticism, especially during a spiritual turmoil caused by a sickness crisis. In my practice, nonbelievers are more prone to seek spiritual explanations to fill the psychological and spiritual voids in their nontheistic or atheistic worldviews.

Theory of Human Caring

According to Alharbi and Baker (2020), Watson’s Theory of Caring is based on the premise that people cannot be managed as objects and that humanity cannot be disconnected from self, others, surroundings, and the world. Watson’s approach spans the whole nursing practice, focusing on the interpersonal interaction between nurse and patient. The idea emphasizes the importance of caring and caring-to-caring humanistic connections and their therapeutic significance for both the patient and the nurse.

Nursing entails accepting Jean Watson’s Caring Science hypothesis. My personal philosophy of practice is uniquely equipped to be the center of healing, according to the theory of human caring, which states that the purpose of a nurse is to place her patients in the greatest position to self-heal (Watson & Woodward, 2020). In line with the notion of caring, I increase my patient’s ability to heal from within by personally partaking in caring via authentic presence and commitment. Example Of A Past Experience Concerning My Worldview

The Nursing Process and Worldview Personal Statement experience occurred as a first-year university student. I came across a patient who had stage 2 colon cancer and significant liver cirrhosis. I tried to explain that his illness was still in its early stages and that he needed treatment as well as a reduction in alcohol use. I reassured him that he could still lead a regular life despite undergoing cancer treatment. He kept referring to God and asking why God was doing this to him despite his efforts and donations to humanitarian organizations. He went on to say that he had adopted two orphans and fostered a total of six children and that with all of his good works,

God should not be punishing him with chronic illness. Because he refused treatment, I asked him if he wanted a visit from a priest, his dogs, and his children, and he accepted. He decided to undergo chemotherapy after the clergyman and his children persuaded him not to give up hope. Three years later, I ran into him, who was still alive and well. He now tells me that if I had not attended to his spiritual, religious, and cultural needs on that initial visit, he would have died.

In the book Jean Watson’s Theory of Human Caring, Watson and Woodward (2020) explain the theoretical application of patient contentment as one of the known objective indicators of medical system performance and effectiveness and my nursing activities. Satisfaction is linked to patient safety because it impacts future healthcare usage as well as the amount of adherence or compliance with recommended treatments, regimens, and recommendations (Watson & Woodward, 2020).

My  Worldview concerning Watson’s Theory Of Human Care

Watson’s theory, in conjunction with my worldview in nursing practice, will assist me in developing my future practices by responding to and becoming attentive to each of my patient experiences – spiritual, cultural, and religious (Wei & Watson, 2019). Through caring science, I will convey compassionate and loving recognition that I can be an outstanding nurse and a genuine accomplice in my patients’ health. What are some of the major ethical issues in conducting research that impacts the advanced registered nurse? Discuss the role of the advanced registered nurse in advocating for patient safety and rights in conducting research.

Study ethics entails standards for the research process, subject dignity safeguarding, and disseminating research findings. This is also true in research, where nurses should contend with three societal values: community, healthcare, and science, which collide with the beliefs of people and societies, resulting in challenges in nursing research. The following are the primary ethical concerns when performing research: Informed consent, Beneficence, respect for confidentiality, and respect for privacy.

Informed consent is a fundamental ethical concern in research. According to Gundugurti et al. (2022),  it suggests that a person gives his consent deliberately, willingly, cognitively, and in a plain and transparent approach. Informed consent protects a patient’s right to autonomy. Gundugurti et al. (2022) define autonomy as the capacity to operate individually in accordance with a personalized plan. Informed consent seeks to accept the rights of independent individuals via self-determination. It also seeks to protect individual autonomy and honesty and avoid attacks on the patient’s dignity. The ethical concept of beneficence relates to being of benefit and not causing damage.

Fleming and Zegwaard (2018) assert that the beneficent principle involves the professional obligation to do effective and meaningful research to serve and enhance the welfare of participants effectively. According to Gundugurti et al. (2022), beneficence refers to the advantages of the research, whereas non-maleficence refers to the risks associated with participation in the research.  Non-maleficence requires a high level of sensitivity from the researcher about what constitutes damage. Gundugurti et al. (2022) state that harm to the participant can be physiological, emotional, social, and economic in character.

The APRN should advocate for the study participants while working with persons engaged in research (Heck et al., 2022). This necessitates some understanding of participant rights and the safeguards in place for human beings in clinical research. The nurse advocates for the study participants by ensuring that they comprehend the experiment and what they are being required to do, that they obtain the knowledge they need to make knowledgeable decisions before and during the study, and that they are aware of their right to withdraw without penalty.
