My clinical area of interest is treating treatment-resistant depression. Working in behavioral health has given me an introduction to this topic, and the treatments that have recently been introduced are fascinating. Some of these current methods are trauma-focused therapies, eye movement desensitization and reprocessing (EMDR), transcranial magnetic stimulation (TMS), ketamine infusions, and psilocybin.
A psychiatrist I work with has recently opened a ketamine clinic and is doing clinical research on psilocybin at the University of Arizona. I recently found a podcast on psychotherapy and listened to a two-part, four-hour episode on the efficacy and research on using psilocybin to treat treatment-resistant depression. I first went to Psychology Databases Combined Search through the Walden Library and searched “treatment-resistant depression.” I came up with nearly 4,000 results. I then narrowed down the years of publication from 2016-2021, which brought up roughly 1200 articles. Perhaps I was too generic in my search. Using our class resources, I found through Walden University Library (n.d.- b) that I could use increase the rigor and effectiveness of my search using advanced search options to select “systematic review” and/or “randomized control trials” within the search databases. There were also options to select quantitative versus qualitative amongst many other options. This led me to a wealth of interesting and relevant articles on my chosen topic.
Walden University Library (n.d.- c) also gave me a framework for developing a PICOT (patient, intervention, comparison, outcome, and time) question. The format is: “In _______(P), what is the effect of _______(I) on ______(O) compared with _______(C) within ________ (T)? In the aged population, what is the effect of exercise programs on accidental falls, as compared with no exercise?” My PICOT question became: In patients with treatment-resistant depression, what is the effect of treatment (TMS, ketamine, psilocybin etc.) on depression remission or lowered rates of suicidality compared with standard depression treatment within 3-6 months. Leadership provides the direction to workers on the most appropriate ways to attain organizational objectives. Leaders have a number of roles which include, but are not limited to tasks which include planning, creating and implementing techniques, motivating workers, enhancing a competitive advantage and developing goals as they meet those that have been set.
Sfantou and his associates (2017), adds that leaders should have the ability to benefit off strengths and have the ability to assess weaknesses as they find ways to mend them. Some of the leadership behaviors used in the workplace involve discovery skill questioning.
The use of discovery skill questioning can assist an individual by assisting them get an appropriate answer to resolve his or her issue. The area of weakness one foresee in the discovery skill is not getting to the point of one’s question and a language barrier.
At times leaders need to sit back and observe. One should be keen when doing this since it can be perceived by others as a weakness which one does not understand. The way to enhance this area is to ensure that you are always engaging while observing. An example is Steve Jobs Amazon CEO who used the skill to develop and manage an effective workplace in the firm (Gregersen, 2015). Effective leadership can make all the difference for employees in the workplace. What works for some employees doesn’t work for others. However, I learned via a study done by Delay and Clark (2020) that there was a large positive effect size in a study of 10,000 magnetic resonance techs that showed a preference for transformational and transactional leadership styles, while laissez-faire leadership style negatively correlated with job satisfaction. The transformational leader resonates the most with me.
Reading Kiwanuka et al. (2021) gave me an “aha” moment when they said that transformational leadership consists of “leading by example, communication, ability to think outside the management square, knowing your staff, and stepping up during times of crisis” (para. 17). We currently have an abundance of nursing supervisors at my work. When I held the position, I carried a full patient load, did all of the supervisor duties, and was alone. Since I stepped down, they now have two supervisors on duty, and neither carries a patient load. I would describe the majority of them as having a laissez-faire leadership style. Most of the floor nurses don’t respect them.
However, one stands out. She works harder than anyone else on the unit. She is always there when you need her. She will offer to take vitals and medicate your patients if you are busy with a crisis. She will take over a petition or voluntary patient transfer to another facility so that we can focus on our patient load. She leads by example, she knows her staff, she doesn’t always care what corporate’s needs are, but does care about our needs, and she steps up in times of crises. However, her transformational leadership style comes at a cost in the workplace. She works longer hours than anyone. She comes in on her days off when we are short-staffed. She burns herself out. Although her leadership style motivates many of us, it also allows other nurses to slack off because they know she will take care of loose ends. In this regard, her leadership style is not always effective and can create a chaotic work environment when she storms through like a tornado trying to do everything at once.
For those of us that work harder and find inspiration from her leadership style, it works well. However, there may need to be a mix of styles to motivate all employees effectively. Foster (2021) says that sometimes that mix must come with a stern, commanding leadership style. However, to maintain employee trust and respect, a commanding leadership style must also be tempered by kindness and that the best leaders, regardless of their style, will always “show respect, compassion and kindness towards their staff” (para. 8). Leaders are human. They aren’t perfect, and they can’t all be placed perfectly into a labeled box with a particular leadership style. From the research I looked at, I lean towards a predominantly transformational leader. However, a mix is necessary when working with a large group of individuals.

The goal of the Strengths Finder Assessment is to help individuals identify their strengths. To identify strengths each participating individual takes an assessment that offers two statements, and the reader chooses the option that is most right to them. At the end of the assessment, the top five themes are identified. Paired with the top five are ten ideas for action to home in on and harness the areas identified as strengths (Rath, 2007). My top five themes included input, intellection, responsibility, consistency, and harmony.
Core values are the values we live by each day. Many different professions and institutions have core values they hope to perform their job or run their business by. Core values for a person are unique and individual. As a profession values most important to nursing have been identified as human dignity, integrity, altruism, and justice (Schmidt & McArthur, 2017). Responsibility and consistency are two values that I would agree are part of my identity and personality. Responsibility for a nurse is deep-seeded, we are not just responsible for ourselves, and our home lives when we arrive to work we carry tremendous responsibility on our shoulders. As daily leaders in our work capacities, we have the responsibility to represent professional behaviors (Davis, 2017).
To strengthen responsibility as a core value in nursing is to care for the patient, their lives, the family waiting for them to come home, and doing so in a kind, compassionate manner. Consistency is strengthened as core value by showing up every day prepared to complete the job to the best of your ability. In nursing responsibility and consistency are connected and important.
Input is definitely a term that describes me. If you asked my husband or colleagues the number of questions I ask they might describe me as leaning toward obnoxious. I have always been a very inquisitive person so nursing really felt right in that regard.
Why, because nurses don’t do interventions without understanding the rationale. One recommendation from the assessment was to find an avenue to share the information I am learning or taking in, I think at some point teaching might be an option. Week 5 Discussion: Leadership Profile

The purpose of the leadership profile is really to help a leader gain understanding of what leadership entails, and also for one to examine the effectiveness of their leadership skills (Rosenbach & Sashkin 2014). As a nurse leader, it is very important for one to understand their leadership profile in order to hone in on their strengths as well identify areas that need improvement on. The Gallup StrengthsFinder assessment revealed the five themes of my strengths as a leader in this order: harmony, empathy, learner, context, and restorative (Gallup, 2021). The results do not surprise me because they are very similar to the results of my business chemistry assessment that I took a couple of years ago. Business chemistry is designed to enable leaders and team members understand similarities and differences in workstyles in order to foster better work relationships & environments and reduce workplace conflicts (Deloitte n.d).
Similar to the StrengthsFinder, there are four types of business chemistry types – pioneer, driver, integrator, and guardian. My top two styles were integrator and guardian. Integrator is very closely aligned with my harmony and empathy in that integrators love team building, are big on empathy, and forge strong relationships. Guardians which somewhat aligns with context and restorative themes tend to be reserved but very loyal and are very methodical and meticulous with tasks (Deloitte n.d).
The nursing profession is one that is rooted in ethics and core values (Poorchangizi, Borhani, Abbaszadeh, Mirzaee, & Farokhzadian 2019). These values keep us grounded and help us make ethical decisions that impact patients on a daily basis. The two core values that I would like to strengthen based on my results are dependability and consistency. I believe that these are values that can always be improved upon (Bean- Mellinger, 2020). Being dependable and consistent provides stability in the work environment.
It simply means that one’s team can count on one to show up, be committed, and follow through at all times. As someone that manages a small team at work, it is important for me to set the tone of dependability and consistency so that it can trickle down to the team members – thus improving the overall posture and morale of the team. The two strengths from the StrengthsFinder assessment that I would like to improve upon are context and restorative. According to Gallup (2021), individuals who are strengthened in the context theme are very interested in how history helps shape current and future affairs. Furthermore, these individuals typically enjoy spending time with experts with philosophical views of the past in order to understand the what, when, who, how and where of events (Gallup 2021).
I think this is an excellent trait to develop especially in our field of nursing as it helps cultivate a questioning attitude which leads to a clinical question, and ultimately leads to research studies that produce evidence based clinical practices.
From a restorative theme perspective, Gallup (2021) states that these individuals are great at figuring out what is wrong and resolving it. In addition, these individuals tend to conduct root cause analysis on issues so as not to have them repeat themselves. Furthermore, these individuals meet deadlines and are fulfilled when they actually complete a task before the due date (Gallup 2021). This description fits me so well – one way I can improve on the restorative theme is by documenting lessons learned from any project, and coming up with action plans for process improvement. The health care field is one that is constantly evolving, so it is important for one to stay current, identify what does not work, and implement strategies to improve or build upon an existing process. The two characteristics that I would like to strengthen are adaptability and optimism. As mentioned earlier, the field of health care is always evolving and the only constant is change (Mira, 2015). As a health care professional, I must learn to be flexible enough to adapt to the many changes that occur. For example clinical protocols that applied yesterday may become obsolete today. Having a mindset of “this is how we have always done it” or being resistant to change can negatively impact growth and development, and inadvertently impact patient health.
Optimism on the other hand is something one can never have too much of. In a world and in a profession where one sees death, poor prognosis, or encounter really upsetting patient cases, it can be very physically and emotionally demanding and draining. Have an optimistic outlook on life in general can improve a nurse’s level of function as well as patients’ satisfaction (Boldor, Bar-Dayan, Rosenbloom, Shemer, & Bar-Dayan (2012).
As nurse leaders, it is important for us to understand our leadership styles, hone in on our strengths, and improve on our weaknesses. It is not enough for us to have the hard skills but more importantly, we need to develop the soft skills (such as emotional intelligence) needed to be transformational leaders that have positive impacts and make a difference in society as a whole.
