Questions like these are related to the underlying pharmacokinetic and pharmacodynamic processes of pharmacotherapeutics. As an advanced practice nurse, understanding these fundamental pharmacotherapeutic concepts is important to ensure that the prescription drugs you recommend for your patients will be safe and effective to treat and/or manage their symptoms. Additionally, as the advanced practice nurse, it is your responsibility to ensure that when prescribing prescription drugs, you adhere to the ethical and legal principles set forth for prescribing drugs as an added layer of protection and safety for the patients you will treat. Discussion: Pharmacokinetics and Pharmacodynamics

As an advanced practice nurse assisting physicians in the diagnosis and treatment of disorders, it is important to not only understand the impact of disorders on the body, but also the impact of drug treatments on the body. The relationships between drugs and the body can be described by pharmacokinetics and pharmacodynamics.

Pharmacokinetics describes what the body does to the drug through absorption, distribution, metabolism, and excretion, whereas pharmacodynamics describes what the drug does to the body.

When selecting drugs and determining dosages for patients, it is essential to consider individual patient factors that might impact the patient’s pharmacokinetic and pharmacodynamic processes. These patient factors include genetics, gender, ethnicity, age, behavior (i.e., diet, nutrition, smoking, alcohol, illicit drug abuse), and/or pathophysiological changes due to disease.

For this Discussion, you reflect on a case from your past clinical experiences and consider how a patient’s pharmacokinetic and pharmacodynamic processes may alter his or her response to a drug.

Patient Case: Opioids and Addiction

          Every day, roughly 128 persons in the United States die as a result of an opioid overdose (NIH, 2020). This suggests that between 21 and 29 percent of patients who use opioids for chronic pain are abusing them (NIH, 2020). In 2018, I looked after a woman in her thirties. I spent some time getting to know this woman because I had to care for her for several days. Her stories about her drug addiction and how embarrassed she was to have put her family through what she did were widely circulated. She said that she never intended to get addicted and that she began taking opioids to relieve pain from a vehicle accident.

Pharmacokinetics and Pharmacodynamics

            Methadone is widely used for the treatment of opioid dependence. When speaking of pharmacokinetics, methadone is a liposoluble drug (Neto & Garcia, 2015). It is a synthetic long-lasting opioid (Neto & Garcia, 2015). When administered orally, this medication has a fast and almost complete absorption (Neto & Garcia, 2015). Therefore, it can be detected in the plasma thirty minutes after oral dosage it completed (Neto & Garcia, 2015). The half-life of methadone is approximately 24 hours and it contains 80-90% plasma binding (Neto & Garcia, 2015). This is especially helpful when it comes to addicts because it is less susceptible to tolerance than other drugs (Neto & Garcia, 2015). Methadone is metabolized by the liver and excreted by the kidneys (Neto & Garcia, 2015). Therefore, the weight of the patient could be an influencing factor on pharmacokinetics.             When speaking of pharmacodynamics, Methadone is considered an opioid-agonist (Neto & Garcia, 2015). Its pharmacodynamic properties include analgesia, respiratory depression, dependence, and even tolerance (Neto & Garcia, 2015). However, studies have shown that this opioid is less sensitive to tolerance since it was not altered after exposure to morphine (Neto & Garcia, 2015). In addition to this, Methadone promote strong serotonin and norepinephrine uptake in the central nervous system (Neto & Garcia, 2015). Considering the combination of these properties, this makes methadone a useful tool to manage pain, tolerance, and addiction to opioids (Neto & Garcia, 2015). The processes of pharmacodynamics in this situation can be influenced by the other drugs that the patient was using prior to Methadone and other drugs she is taking along with Methadone. However, Methadone is a cross-tolerance opioid, which means it blocks the effects of other opioids (CAMH, 2018).

Personalized Plan of Care

            Since Methadone is an opioid and will need to be prescribed long term, frequent and ongoing management would be needed for this patient. The patient would need to either be present in the clinic or available for a telehealth conference at least once a week. Twice a month, the patient would need to be present in the clinic for an appointment. These appointments would be important to make sure the patient is taking the methadone and has the correct dosage prescribed. When methadone reaches a sufficient level in the blood, it will alleviate the patient’s cravings, suppress opioid withdrawal symptoms, reduce the euphoric effects of other opioids, and would not induce sedation (CAMH, 2018). Other medications that the patient is prescribed would be updated at each appointment along with health history to reduce the influence on pharmacodynamic and pharmacokinetic processes. The patient would also be expected to attend therapy sessions along with PCP follow ups to get the prescription of methadone refilled.NURS 6521 Weekly Discussions and Assignment Papers

Conclusion

            Methadone can be a great tool to treat those who are struggling with addiction to opioids. This can be addiction to heroin or opioid prescription pills such and oxycodone. Methadone has a very high plasma binding percentage and half-life. This means the medication can build in the body to create a stabilized environment that suppresses withdrawal symptoms, reduces cravings, and reduces the euphoric effects of other opioids (CAMH, 2018). However, a personalized patient plan would need to be created and continually modified to fit each patient in order for the treatment to be successful. What type of drug should you prescribe based on your patient’s diagnosis? How much of the drug should the patient receive? How often should the drug be administered? These are some of the questions you might consider when selecting a treatment plan for a patient.

As an advanced practice nurse prescribing drugs, you are held accountable for people’s lives every day. Patients and their families will often place trust in you because of your position. With this trust comes power and responsibility, as well as an ethical and legal obligation to “do no harm.” It is important that you are aware of current professional, legal, and ethical standards for advanced practice nurses with prescriptive authority. Additionally, it is important to ensure that the treatment plans and administration/prescribing of drugs is in accordance with the regulations of the state in which you practice. Endocrine system Disorder, Diabetes Mellitus

Diabetes mellitus (DM) is a disorder of the endocrine system that affects how the body uses glucose. The various diseases and the treatments. DM is a group of metabolic disorders where hyperglycemia is a distinctive factor. Patients with DM are often undiagnosed or under-diagnosed; they have an increased risk for evolving life-threatening diseases, incurring substantial medical costs, and a decreased quality of life.  According to Brutsaert (2019), there are different types of DM such as type 1 DM, type 2 DM, and gestational diabetes. Endocrine disorders are common, and the advanced practitioner must be able to distinguish between the glucose levels in DM are abnormally high and the body is unable to make enough insulin to bring down the glucose levels. The purpose of this discussion is to compare DM type 1, type 2, and gestational diabetes while exploring the drug treatments and dietary considerations of gestational diabetes. Comparison of Diabetes Mellitus Type 1, Type 2, and Gestational Diabetes:

    DM type 1 is an autoimmune disease where the pancreas does not produce insulin (Saberzadeh-Ardestani et al., 2018).  According to Brutsaert (2019), DM type 1 is usually diagnosed in childhood but can occur at any age.  DM type 2 is the most common endocrine a disorder where the body is insulin resistance and is unable to produce enough insulin (Mihai, Lacatusu, Grigorescu, & Botnariu, 2015).  According to Brutsaert (2019), DM type 2 is usually referred to as adult-onset diabetes and the body does not produce enough insulin to meet the body’s need resulting in elevated glucose levels.  Gestational diabetes is a complication of pregnancy and results in hyperglycemia due to insulin sensitivity (Plows, Stanley, Baker, Reynolds, & Vickers, 2018).  According to Friel (2019), during pregnancy, more insulin is needed due to the placenta releasing a hormone that causes the body to not respond to insulin resulting in gestational diabetes. Gestational Diabetes Treatment:

   The goal of treatment for gestational diabetes is to monitor blood glucose levels and keep the levels normal throughout pregnancy to ensure the health of the mother and fetus.  According to Friel (2019), the initial treatment plan is monitoring diet and incorporating low impact exercise.  If diet and exercise do not help bring down glucose levels, drug therapy should be started either with insulin or oral agents.  Insulin was always the gold standard of treatment for gestational diabetes, but it has been found to cause increased weight in pregnant women or cause hypoglycemia which has led to the use of metformin (Sindu et al., 2018).  According to Rosenthal & Burchum (2018), metformin is an acceptable alternative to insulin due to the treatment outcomes being the same. 

Metformin:

   Metformin mechanism of action is to reduce glucose levels by activating adenosine monophosphate-activated protein (AMP) a kinase (Sindu et al., 2018).  According to Rosenthal and Burchum, 2018), metformin is absorbed slowly from the small intestine and not metabolized but rather excreted by kidneys.  Metformin is available in immediate-release and extended-release forms and taken by mouth (Rosenthal & Burchum, 2018).  The immediate-release is initially dosed as 500mg twice daily with breakfast and dinner whereas the extended-release is dosed 500mg daily with dinner (Rosenthal & Burchum, 2018). 

Dietary Considerations:

   The goal of treating gestational diabetes is to maintain normal blood glucose by initiating good nutrition (Sindu et al., 2018).  The patient with gestational diabetes will be asked to measure their blood glucose several times a day an hour after eating so proper nutrition ensures better outcomes (Friel, 2019).  The patient should start by decreasing carbohydrate intake is the first while eating small meals and snacks spread out throughout to maintain normal glucose levels. 

Short-Term and Long-term Effects of Gestational Diabetes:

   The short-term effect of gestational diabetes for the woman is the risk of developing pre-eclampsia and potential for a c- section (Sindu et al., 2018).  The short-term effect on the fetus is macrosomia, shoulder dystocia, and hypoglycemia at birth (Sindu et al., 2018).  The long-term effects on the mother are the potential of developing type 2 DM after delivery as well as gestational diabetes in future pregnancies (Friel, 2019).  The fetus may be affected long term by gestational diabetes and be at risk for weight problems and diabetes later in life (Friel, 2019).  According to the study conducted by Sindu et al. Conclusion

The advanced practitioner must understand all the diabetic diseases and the treatment plans that will ensure the patient’s health is maintained. Each diabetic treatment is different; the approach to treatment remains to avoid medications when lifestyle changes can maintain healthy blood glucose levels. The long-term goal is to keep diabetes from affecting the rest of the body, improving the quality of life. Each medical treatment must be individualized to maintain the patient’s blood glucose levels, optimizing the medical regime, and reducing any potential side effects. Holistic treatment must factor in the psychological needs of the patient, monitoring for depression related to newly diagnosed diabetes.

Assignment: Off-Label Drug Use in Pediatrics

The unapproved use of approved drugs, also called off-label use, with children is quite common. This is because pediatric dosage guidelines are typically unavailable, since very few drugs have been specifically researched and tested with children.

When treating children, prescribers often adjust dosages approved for adults to accommodate a child’s weight. However, children are not just “smaller” adults.
