I spent quite a good time with my preceptor discussing the organizational policies. This discussion helped me to come up with the objectives of the project. The objectives then guided the action plan that ultimately enabled me to realize the outcomes of the project. Four outcomes were arrived at guided by the objectives.

To begin with, the first outcome would be a reduction in the incidences of ventilator-acquired Pneumonia (VAP). VAP is a common nosocomial infection that continues to cause mortality and morbidity. On the other hand, chlorohexidine has been used in various settings with good results reported. The application of chlorhexidine (CHG) should help in reducing the bacterial load that would ultimately lead to decreased VAP and a reduction in hospital stay. In addition, resistant strains of organisms would be reduced through proper administration of CHG in all eligible individuals. The second outcome is increased knowledge among nurses about the evidence-based project (EBP).

Empowered nurses have improved cognitive levels that improve their attitude and behavior towards the EBP (Mathew et al., 2022). Furthermore, nurses who understand the importance of the EBP are likely to offer quality services through adherence to daily bathing. This improves patient outcomes as well as reduces mortality. In addition, knowledgeable nurses will teach others and encourage them to adhere to the EBP guidelines to ensure patient safety.

The third outcome would be a reduction in the cost of care. Essentially, following the guidelines reduces the microbial load leading to a reduction in VAP. Patients without complications are likely to recover quickly thus reducing hospital stay that ultimately reducing the cost of care. This is because patients who spend longer times in hospital are likely to develop other complications that would require the use of resources to enable them to get help. Lastly, the general patient outcomes will be improved upon implementing an EBP project. The patient outcome can be measured in terms of morbidity, mortality, length of hospital stays, cost of care, and satisfaction level.

An individual success plan maps out what you, the RN-to-BSN student, needs to accomplish in order to be successful as you work through this course and complete your overall program of study. In this ISP, you will identify all of the objectives and assignments relating to the 100 direct clinical practice experience hours and the 25 indirect clinical practice hours you need to complete by the end of this course. NRS 493 Capstone Project Topic Selection and Approval

In collaboration with the approved course preceptor, students will identify a specific evidence-based topic for the capstone project change proposal. Students should consider the clinical environment in which they are currently employed or have recently worked. The capstone project topic can be a clinical practice problem, an organizational issue, a leadership or quality improvement initiative, or an unmet educational need specific to a patient population or community. The student may also choose to work with an interprofessional collaborative team.

Students should select a topic that aligns to their area of interest as well as the clinical practice setting in which practice hours are completed.

Write a 500-750 word description of your proposed capstone project topic. Include the following:

The problem or issue, intervention, quality initiative, educational need, or collaborative interprofessional team project that will be the focus of the change proposal.

Review your problem or issue and the study materials to formulate a PICOT (Patient, Intervention, Comparison, Outcome and Time) question for your capstone project change proposal. A PICOT question starts with a designated patient population in a particular clinical area and identifies clinical problems or issues that arise from clinical care.

Example Journal Topic 3

This week has been very successful and exciting. I spent most of my time with my mentor to discuss issues around health care delivery and clinical systems; a vital component in ensuring safety and quality of care to patients. Various parameters are addressed in this subject; they include health advancement, diagnosis is illnesses, health promotion and disease prevention, treatment and management of clinical conditions, appropriate rehabilitation plan, and effective communication (Hand et al., 2021). We concluded that improving the healthcare delivery and clinical system (HDCS) is a paramount task that would improve clinical outcomes.

From the interactive session we had, I decided to come up with a PICOT question that would help in improving the HDCS by using nurse driven-protocol to reduce ventilation acquired pneumonia in intubated patients using chlorhexidine. VAP is a common nosocomial infection in the intensive care unit that lead to mortality, increased cost of care, and poor patient outcome (Veet et al., 2020).

My ability to communicate effectively, form interpersonal collaboration, critically think, and embrace teamwork was essential for me to come up with a project that could improve health systems. In addition, I am assertive, goal-oriented, and have good leadership techniques that are paramount in the clinical system. On the other hand, I have personal weaknesses that require improvement. Such include poor public speaking skills, poor time management skills, and procrastination; these could interfere with my nursing skills.

Improving patient outcomes in the ICU requires advancement in the mode of care through the provision of required resources. For instance, there is a need for resource mobilization to install bedside imaging such as ultrasound and radiograph to aid in diagnosis; education of healthcare providers about infection prevention practices to reduce infection rate, and integration of communication systems to ease information sharing and improve patient outcomes. Involving nurses and other healthcare providers will improve care delivery that improves patient satisfaction.
