When conducting research, many ethical issues can arise for the advanced practice registered nurse (DeNisco, 2021). The patient’s decision-making capacity is an example of one ethical issue. In some cases, decision-making capacity is straightforward. For example, a comatose adult patient lacks decision-making capacity, while an adult patient who is alert and oriented and can relay back everything they are told, all while asking important questions, is more than capable of making their own decisions. In other cases, decision-making capacity is difficult, as their capacity is less obvious or the patient’s decision-making ability changes throughout the course of treatment.

According to the President’s Commission for the Study of Ethical Problems in Medicine and Biomedical and Behavior Research, there are three minimal capacities needed to be met by a patient for them to make their own decisions. First, the patient must have personalized goals and values. Second, the patient must be able to communicate with health care staff and prove understanding of information. Finally, the patient must have the ability to deliberate and think about their choices. Furthermore, even if these three things are met, the nurse must evaluate whether other influences are pressuring the patient into making a specific decision. These influences could be external or internal. Another example of an ethical issue in research that impacts the advanced practice registered nurse is a conflict of interest.

A conflict of interest occurs anytime there is pressure to act in a way or do something that does not keep the patient’s best interest as the primary point of care. Conflicts of interest can be environmental, interpersonal, and economic. “Ongoing self-reflection and reflection on nursing practice are crucial, as is remaining aware that conflicts of interest are ever present and may result in subconscious biases that do not serve the patient well (DeNisco, 2021, p. 796). An example of a conflict of interest would be the nurse accepting money or gifts from a company whose latest drug is part of a research study at a teaching hospital. The company would be “buying” or “trying to buy” the nurse’s favor regarding the research study’s results on the drug.

The role of the advanced registered nurse in conducting research is to advocate for patient safety and patient rights (DeNisco, 2021). The nurse themself must possess the ethical values of justice, nonmaleficence, accountability, autonomy, beneficence, fidelity, and veracity. The nurse must have personal integrity and promptly relay all reasonable information related to their medical care to the patient. It is crucial to keep the patient and their best interest at the forefront of any research study. The nurse should be aware of any agent-related or environmental factors that could interfere with their nursing behavior and code of ethics. An example of an environmental factor in the field of research is a conflict of interest. An example of an agent-related factor in research is the nurse’s disapproval of the patient’s choice or response.

The nurse must also obtain informed consent from every patient involved in the research trial. The informed consent must be tailored to specifically meet the patient’s needs to verify that they are informed. Next, the nurse should verify a patient’s decision-making capacity before providing any care or treatment and verify that the patient is not swayed by internal or external factors concerning their medical choices. Finally, the nurse should provide the patient with privacy and confidentiality unless the patient’s actions or words fall within the state’s laws, allowing the nurse to break confidentiality to ensure the safety of other persons or the public. The role of the advanced registered nurse is vital in research regarding patient safety and rights.

Topic 8 DQ 1 Professional Nursing Organizations On The Nursing Profession

A professional culture that nurtures the ideals and ethos of the profession among its members is one of the qualities of a professional organization. This professional culture is frequently fostered and maintained by the acts of professional organizations (Cline et al., 2019). Professional organizations are formed to jointly lobby on behalf of their members and other constituencies, publicly promoting the nursing profession’s basic principles to others.

Professional nursing organizations have clinical, political, and regulatory focus roles.  According to Cline et al. (2019), specialty organizations complement regulatory initiatives in the aspects of health policy and practice guidelines to safeguard the public’s wellness and safety. For example, the National Council of State Boards of Nursing is the primary regulating agency in nursing. NCSBN is a non-profit, self-governing organization administered by a management board and a general council representing 59 member nursing boards.

The NCSBN exists to provide a forum through which organizations of nurses act and consult together on problems of mutual interest and importance impacting public health, safety, and welfare, including the preparation of nursing licensing tests (Dickison et al., 2019). Because the NCSBN prepares the NCLEX RN and NCLEX-PN tests, it plays a significant role in the regulation of nursing licensing in the United States. The NCSBN also provides materials to state boards of nursing to assist them in their tasks of regulating nursing knowledge and training. The NCSBN organization aligns with my goals and worldview because, as a national authority, it delivers national knowledge on regulatory problems affecting nurse practice, such as telemedicine and interstate practice.

Nursing organizations are also organized around a specific clinical specialty area and incorporate political, advocacy, regulatory, and professional goals relevant to the clinical field in their mission. The American Psychiatric Nurses Association is one example of a clinically focused professional group. APNA nursing specialty organization’s objective includes improving the health of mental health patients under their care (Cline et al., 2019). A relationship with a clinical specialty organization guarantees up-to-date practice information for the mental health patient group that the nurse cares for, allowing him or her to optimize the quality of care provided. Standards of practice, evidence-based practice recommendations, research initiatives, procedures, instructional seminars, and other resources are examples of these resources. Cline et al. (2019) show nursing organizations develop a wide range of practice and instructional materials to achieve this goal, and this aligns with my goals and worldview.

Nursing as a profession has a societal obligation, with the explicit goal of improving the nation’s health. Professional nursing organizations fulfill nursing’s commitment to support better health achievements in national and global settings in various ways. The American Nurses Association (ANA) is the most prominent professional nursing organization in the United States, representing its members’ perspectives and needs in many governmental arenas (Ball, 2018). All nurses can join the ANA by joining their respective state nursing organizations. The ANA’s political and legislative agenda has addressed problems such as proper compensation for healthcare, services, access to healthcare, and acceptable nurse staffing ratios, which align with my worldviews.

Professional nursing organizations play an important role in making sure the perspectives of advanced registered nurses are heard, and in supporting nurse specialties in their efforts to expand their scope of practice and their full participation throughout the health care system.
